Overview

Refer a Client to PARC

PARC works closely with community partners, hospitals, shelters, outreach workers, and social service agencies across Toronto. If you’re supporting someone who could benefit from our programs, we welcome referrals from professionals and community members alike.

Who Can Refer?

  • Social workers and case managers
  • Healthcare providers and hospital discharge teams
  • Shelter and transitional housing staff
  • Community outreach workers
  • Friends, family members, or neighbours

Who We Serve

PARC supports people who are experiencing or at risk of the following:

  • Homelessness or housing instability
  • Mental health challenges
  • Substance use challenges
  • Poverty or food insecurity

There are no formal eligibility criteria for most programs. PARC operates on a low-barrier, harm reduction model.

How to Make a Referral

Phone: Call us at (416) 537-2262 and ask to speak with a case manager or intake staff.

Email: Send referral details to kriordan@parc.on.ca—include the person’s first name, the support they’re looking for, and the best way to reach them (with their consent).

In Person: You’re welcome to accompany someone to the Drop-In at 1499 Queen Street West. Our team will take it from there.

What Happens After a Referral?

Our staff will connect with the individual, complete an informal intake, and link them to the most appropriate programs or external services. We follow up with referring partners when consent is given.
