Case Management
The Case Management team offers a wide range of comprehensive services to individuals with serious mental health conditions, as well as their significant others. These services include personalized assessment, planning, counseling, service coordination, and advocacy.
The goal of the Case Management team is to connect members to a network of care, stabilize their housing, end their court involvement, reduce hospital visits, ensure entitlement to benefits, enhance social inclusion, and decrease mental health symptoms.
The team’s ultimate objective is to help individuals manage their symptoms and financial circumstances without ongoing case management support.
PARC’s Case Management team receives referrals through the City of Toronto’s Central Access, the Ministry of Health and the Access Point. PARC cannot receive referrals directly.
