
Overview: The Case Management team provides comprehensive services to individuals experiencing homelessness, mental health, and substance use. These services include personalized assessment, planning, service coordination, and advocacy. The case management team connects members to a network of care to meet their individual needs. By stabilizing housing, reducing hospital visits, ensuring entitlement to benefits, enhancing social inclusion, improving mental health symptoms, and navigating legal systems, Case Managers support people to rebuild their lives.
Who it helps: Individuals 18+ who are experiencing issues with mental health, substance use
and homelessness.
How to access it: PARC’s Case Management team receives referrals through the City of
Toronto’s Coordinated Access, the Ministry of Health, and the Access Point.
