Case Management
The Case Management team offers a wide range of comprehensive services to individuals with serious mental health concerns and addictions. These services include personalized assessment, planning, counseling, service coordination, and advocacy.
The goal of the Case Management team is to help connect members to a network of care in order to meet individual needs by stabilizing their housing, reducing hospital visits, ensuring entitlement to benefits, enhancing social inclusion, decreasing mental health symptoms and navigating legal systems.
The team’s ultimate objective is to help individuals manage their symptoms and financial circumstances in order to provide autonomy and freedom.
PARC’s Case Management team receives referrals through the City of Toronto’s Coordinated Access, the Ministry of Health and the Access Point. PARC cannot receive referrals directly.
