Case Management

The case management team is supporting members with daily telephone contact and focused support for those with higher critical well-being needs.

PARC members have cited challenges finding the right mental health care, maintaining housing, accessing benefits, criminal justice support, and building personal Recovery Goals.

Our Solution

We provide one to one support for people who struggle with serious mental health challenges. Our services promote independence and encourage members to build a circle of care that meets their needs.

Our members' common goals include:

Eligibility Requirements

Can You Apply

Must be referred from The Toronto Mental Health and Addictions Access Point.

    How To Apply

    To apply to receive PARC’s Case Management services, please contact The Toronto Mental Health and Addictions Access Point, referred to as The Access Point.

    It's a centralized point where you can apply for individual mental health and addiction support services and supportive housing.

    Case Management is available Monday through Friday, from 9 AM – 5 PM.

    Program Details

    We use a strengths perspective and incorporate harm reduction and anti-oppression values in our work.

    PARC’s Case Management Pprogram is about building trusting relationships between members and our team of community mental health workers.
